I am writing to report a serious safety defect regarding the Driver's Side LED headlight assembly on my 2021 Chevrolet Tahoe (VIN: [XXX]. While driving, the headlight suddenly dropped completely out of alignment and is now pointing directly at the ground, casting light only a few feet in front of the bumper. Upon inspection, it was determined that the internal plastic headlight adjuster mechanism completely snapped/failed. This failure has created an immediate and severe night-driving hazard. Because the LED headlight now illuminates only the ground immediately in front of the vehicle, forward visibility on the driver side is effectively reduced to zero at highway speeds. This drastically r…
The suspension system on the 2022 Tahoe has been defective. It started as a creaking noise we took for repair, the dealership said it was a strut. Creaking noise continued took to another shop they said creaking came from ball joints being bad. The service transmission light comes on and off, service safety restraints, screen goes black and does not respond. Take to dealership multiple times to fix issues but they still continue
The water pump failed. Platinum Chevrolet has the old one as they have replaced it. I was driving in [XXX] on a crowded street when I had to quickly pull over because of a terrible noise and the smell of leaking coolant. It made it dangerous for me to get to a parking spot in traffic without warning. There were no warning symptoms. Had to have it towed to Platinum Chevrolet in Terrell, Texas, which did the inspection and water pump replacement. $1600.00. No clue what caused it other than bad workmanship. I got in my vehicle and turned it on. Once on, it asked me to roll down my passenger side window and roll it back up. I thought that was odd, but did so. My vehicle had completely reset. The clocks were not correct and none of our controls were set. I reset everything, but thought that was weird. A few days later, my husband and son were coming home from a 5-hour trip and as they were turning into our driveway, thank God they were home, the vehicle completely turned off and shut down. The brakes locked up and they came to a complete stop. We lived on a really busy road and this could have been extremely dangerous. My husband quickly turned the vehicle back on and parked in the driveway, but …
Began to accelerate to pass a slower vehicle on the highway. Just as it started to accelerate the engine abruptly shut off and the vehicle began decelerating. Many warning messages came up on the dashboard but none were present prior to the event. Was able to coast onto the shoulder of the highway before stopping. Attempted to restart vehicle but it would not restart. Vehicle was towed to the nearest Chevrolet dealership where they were able to assess that something inside the motor had failed and caused it to lock up.
The contact owns a 2022 Chevrolet Tahoe. The contact stated that while driving approximately 70 MPH, the vehicle stalled with the message "Press to Start" displayed. The vehicle was parked on the shoulder of the road and failed to restart. The vehicle was later towed to the local dealer, and the contact was informed that the engine needed to be replaced. The contact stated that the engine was the replacement engine after the failure of the original engine. The replacement was performed under NHTSA Campaign Number: 25V274000 (ENGINE AND ENGINE COOLING). The manufacturer was notified of the failure. The failure mileage was 10,000.

General Motors has decided to conduct a Voluntary Emission Recall. Certain vehicles may have a condition which can cause the fuel pump power control module to fail. If this were to occur, fuel flow to the engine could be interrupted. Dealers will replace the fuel pump power control module.
This preliminary information communicates an issue with of a service theft message after using the rear window defogger. Engineering is currently investigating.
This preliminary information communicates an issue with a service column lock message and DTC U3000. Engineering is working on the issue and in the meantime dealerships are to reset/clear the DTC and perform four sleep cycles to verify the DTC does not return.
The Rear Camera Mirror display may appear angled downward. Investigation found that an aftermarket or dealer-installed rear license plate frame can contact the license plate lamp applique, pushing the applique upward and skewing the camera display angle. Remove or reposition the interfering frame, verify proper applique seating, and confirm the Rear Camera Mirror display is correctly aligned.
This PI communicates the possibility of a B1599 setting and directs to contact TCSC.

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
On January 16, 2026, the Office of Defects Investigation (ODI) opened Recall Query RQ26001. This investigation was opened to assess the remedy effectiveness of Recall 25V-274. ODI has received complaints alleging engine failure in vehicles after the recall remedy had been completed, whether that remedy involved an oil viscosity change or a complete engine replacement. General Motors (GM) issued Recall 25V-274 to address concerns of engine failure in the L87 engines produced between March 1, 20
The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019â2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 20

Source: NHTSA Office of Defects Investigation public data (complaints, manufacturer communications, investigations), model years 2010 and later. Had a safety problem? Report it to NHTSA.
In complaints filed with NHTSA for model years 2010–2027, owners most often name: unknown or other, engine, air bags. 3,649 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
