In the owners’ own words, as filed with NHTSA. Unverified reports; NHTSA removes personal details before publishing.
The contact owns a 2018 Western Star 5700. The contact stated while driving 70 MPH, the On Guard radar system became inoperable. The contact stated that the feature independently activated the brake. The contact stated that the failure had been ongoing since 2018. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact stated that he contacted On Guard, but no assistance was provided. The manufacturer was made aware of the failure. The failure mileage was approximately 700.
We ordered a Western Star WITHOUT collision mitigation and when we got the truck it had a WABCO CMS. We have had nothing but problems of hard breaking on overpasses and bridges and false readings of cars on the other side of highway. The truck has been in FYDA Freightliner for about a month total and they "can't find anything wrong". Their suggestion was us to remove a bullguard and drive a few runs without it. The truck is inconsistent with it's safety feature, which again we have complained about since day ONE since we didn't want it to begin with.
The contact owns a 2020 Western Star 5700. The contact stated that while driving approximately 75 MPH, the anti-collision system erroneously activated causing the commercial truck to abruptly stop while driving on the freeway. During the failure, there were no other vehicles or objects nearby to cause the system to be activated. The contact indicated that the failure had occurred on several occasions. The vehicle was taken to the local dealer who realigned the sensors however, the failure recurred. No further information was available. The manufacturer was notified of the failure. The failure mileage was 3,000.
TL* THE CONTACT OWNS A 2019 WESTERN STAR 5700. WHILE THE VEHICLE WAS PARKED, THE CONTACT WAS CLEANING THE RIMS AND NOTICED THAT THE LUG NUTS WERE LOOSE ON THE INNER TIRES. IN ADDITION, ALL THE VALVES WERE DETACHED FROM ALL FOUR INNER TIRES. THE CONTACT CALLED AN INDEPENDENT MECHANIC AND WAS INFORMED THAT THE LUG NUTS WERE NOT TIGHTENED PROPERLY, WHICH CAUSED THEM TO LOOSEN. THE VEHICLE WAS REPAIRED. THE VIN WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 18V353000 (SERVICE BRAKES, HYDRAULIC). THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 4,000.
Repair instructions and other communications the manufacturer sent dealers about known issues. A TSB is not a recall, and the repair is usually free only while the car is under warranty; ask your dealer for the bulletin number.

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
On May 26, 2023, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23010) to assess reports of false positive activation of the Automatic Emergency Braking (AEB) system in model year (MY) 2017-2022 Daimler Trucks North America (DTNA) vehicles. The reports alleged that activation of the AEB system occurred while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. DTNA indicated that it received of a total of 315
The Office of Defects Investigation (ODI) received a petition on November 21, 2025, requesting a defect investigation to “determine whether the absence of a redundant mechanical wheel-fastener locking mechanism constitutes a safety-related defect in motor vehicles and motor-vehicle equipment” in the heavy-vehicle sector. On March 4, 2026, NHTSA opened Defect Petition DP26002 to evaluate the petitioner's request. The petition itself can be reviewed at NHTSA.gov under ODI number 11703245. The

On April 23, 2018, the National Highway Traffic Safety Administration (“NHTSA”) opened Audit Query (AQ18002) to assess the timeliness and scope of four recalls filed by Daimler Trucks North America, LLC (“DTNA”). NHTSA subsequently expanded AQ18002 to examine the timeliness and scope of three additional DTNA recalls and compliance with early warning reporting and manufacturer communications reporting requirements. In complaints filed with NHTSA for model years 2010–2022, owners most often name: forward collision avoidance, service brakes, service brakes, hydraulic. 4 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
