I am reporting a severe vehicle safety violation involving a Westlake tire on a 2018 Toyota Yaris rented through Turo (Claim 1060794). The vehicle host rented out the vehicle with a pre-existing, structurally ruptured tire sidewall bulge on the front passenger wheel. The host took a pre-trip photo documenting the failing tire with a gauge but failed to ground the vehicle, choosing instead to temporarily inflate it with air to mask the low-pressure warning light. The tire structurally failed during operation, forcing a replacement. A formal safety investigation is requested into this safety failure for deploying a non-roadworthy, structurally compromised vehicle onto public roads. TIRE …
Tire size: P 175 / 65R 15 84 H I purchased these tires through Walmart in 2022. Now, all 4 of my tires have had multiple cracks in the sidewalls. The front passenger tire was flat when I went to drive, and was replaced with a new tire of the same kind on XXX. I donât feel safe driving with these tires knowing they can deflate at any given moment. There should be a recall issued by the manufacturer. The mechanics at Walmart Auto said that this is not normal for [XXX] tires, and Iâm looking for a way I can be refunded. The infotainment display glass on my 2017 Toyota Yaris iA has a known adhesive defect that causes white "spider cracking" around the edges of the infotainment display and "ghost touches," where the touchscreen registers inputs without being touched. This can change music, navigation, phone, or menu settings while driving, creating a distraction and safety hazard. The issue occurs unpredictably and cannot be disabled by the driver, nor can the infotainment display be powered off as a solution. Mazda settled a lawsuit (Duffy, et al. v. Mazda Motor of America, Inc.) and issued Service Alert SAâ041/22 for this exact defect on the Mazda2, which uses the same Mazdaâbuilt infotainment hardwa…
Vehicle was in Park and keys were out and I was on passenger side unloading a shelf and the shelf hit the shifter and the vehicle started rolling backwards and ran over my leg.
Over corrected, hit a tree, flipped car 2x landed upside down white smoke started filling in the car and went completely engulfed in flames within a few minutes.
The vehicle do unnecessary abrupt breaking in the highway when the road are reduced due to reparation. In addition in two times, when I tried to park to my garage in the back of other vehicle the breaking system related to collision is activate. This area of my garage is an inclined drive way zone and I put the vehicle back to other. In occasions when I go out to my residence, I need to go thru a control system that content a electrical barrier (electric arm), and around 4 or 5 times, when I close to this area the car did a hard stop, and the collision red light turned on with a beep sound, the same indication when the car activate the breaking system in the highway and when I try to park in…
I was involved in a accident where the airbags did not deploy. I was hit on my driver's side and the vehicle was totaled but the airbags never deployed. I also dealt with electrical issues the entire time I had the vehicle. I believe the electrical issues cause the airbags not to deploy. I did receive the vehicle in that condition.

When certain vehicle ECUs are replaced, the ECU Security Key must be written in order for that ECU to communicate on the vehicle network. The GTS+ Software Health Check Results screen has been enhanced to help identify if a replacement ECU requires an ECU Security Key.
The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation
To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint,
Toyota vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination. Follow the Removal Procedure in this bulletin to remove the RapgardTM protective film within 90 days from initial application.
Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid. Follow the Repair Procedure in this bulletin to clean vehicles that may have been subjected to contamination by airborn

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) received a petition dated June 7, 2023, along with several subsequent submissions from the petitioner, requesting an investigation of the vehicle CAN Bus vulnerability to external attack in the Model Year (MY) 2019 Toyota Yaris. The petitioner described multiple electrical symptoms including but limited to: multiple electrical instrument panel warning lights, inaccurate fuel gauge, inoperative
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 20
Source: NHTSA Office of Defects Investigation public data (complaints, manufacturer communications, investigations), model years 2010 and later. Had a safety problem? Report it to NHTSA.
In complaints filed with NHTSA for model years 2010–2026, owners most often name: air bags, electrical system, vehicle speed control. 260 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
