The 2023 Toyota Prius Prime features fixed, non-adjustable rear suspension geometry. My vehicle arrived from the factory with the Right Rear Camber out of specification (-2.2°, max spec limit is -2.0°). Because the rear camber is non-adjustable and cannot be corrected, it caused rapid, unseen inner tread wear down to the steel cords by 20,000 miles, creating a severe tire blowout risk. Authorized Toyota dealership Ourisman Fairfax Toyota inspected the vehicle under RO #6195543/1 and verified that the camber is out of spec and that there is NO external physical damage to the suspension. Toyota Corporate closed my case and refused to address or reimburse the defect under the 3yr/36k Basic Wa…
Incident: On Sept 12, 2026, my 2024 Toyota Prius Prime continued moving forward after I pressed the Park button, causing a collision with my condo garage wall. This was unintended vehicle movement after Park engagement. The vehicle uses a pushâbutton, shiftâbyâwire Park system, so any electronic malfunction can affect its ability to remain stationary. Recent recall work: Two days before the incident (Sept 10, 2026), Toyota performed recall work on the vehicle. Because the Prius Prime relies heavily on electronic control modules, this timing may be relevant to the malfunction. Same day: The vehicle displayed âNo Key Detectedâ even though the key fob was inside. This may indicate i…
The vehicle exhibits an unaddressed factory programming defect where the Plug-In Charge Control ECU fails to enter sleep mode while plugged in or resting. This causes continuous, heavy parasitic drain on the 12V auxiliary battery, causing chronic low-voltage state across critical systems. The 12V auxiliary network supplies power to the primary control units, CAN-bus communication modules, and dynamic stability systems. Operating under chronic low-voltage causes system instability, erratic sensor processing, voltage brownouts, and rapid degradation of the 12V battery. In severe traffic or isolated operations, a depleted 12V battery creates a distinct safety hazard, including sudden loss of pr…
My 2023 Prius Prime cannot be charged normally in hot weather without triggering a thermal-protection fault on its high-voltage charging system. Avondale Toyota diagnosed it in writing: "CODE P1CE996 CHARGER COOLING FAN REVOLUTION SENSOR COMPONENT INTERNAL FAILURE. THE ELECTRIC CHARGER ASSEMBLY HAS A INTERNAL OPEN CIRCUIT CAUSING IT TO BE INOP. RECOMMEND REPLACEMENT ADD PN# G9090-47061." Quote: $2,516.99 plus ~$200 diagnostic. The vehicle cannot verify its high-voltage charger's cooling fan is operating. In Phoenix heat (100F+), normal charging repeatedly sets the fault and check engine light. I verified by controlled testing: reduced-current charging in cool hours avoids the fault, but any …
The contact owns a 2017 Toyota Prius Prime. The contact stated that while driving at an undisclosed speed, the vehicle stalled. Several unknown warning lights were illuminated. The contact stated that while attempting to start the vehicle, the vehicle started. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The contact used a portable diagnostic machine to diagnosed the vehicle; however, no fault codes displayed. The contact stated that the failure had not reoccurred. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The manufacturer was not made aware of the failure. The failure mileage was…
The contact owned a 2024 Toyota Prius Prime PHEV. The contact stated that while driving at 40 MPH, the vehicle was involved in a head-on collision with another vehicle that was traveling towards the contact's vehicle at approximately 50 MPH. The contact stated that during the impact, the steering wheel detached and hit the driver's body. The air bags failed to deploy. There was no warning light illuminated after the crash. The vehicle went into Crash mode and automatically connected with OnStar, which called emergency units. The contact stated that the entire front ends of both vehicles were destroyed. Both vehicles were towed to an impound lot, where they were totaled. The contact's vehic…
In the 2017 Prius Prime when the car switches from Electric power to gas the car frequently fails to allow you to accelerate. It is like you are stuck in neutral on the freeway, taking your foot off of the acceleration pedal and pushing it down doesn't always fix the issue. The accelerator foot pedal can be completely down to the floorboard as you are losing speed. The safety issue is that you are losing speed, and on the freeway and the car doesn't recover losing some control. I have taken the car to the Toyota delearship and when taken in that say it is a software update, but this continues to happen. There is no real notification when this will occur as the battery milage switchover is o…
This is a 2024 Toyota Prius prime lease in February 2024. I went to our trusted local car shop for our annual inspection, and was told that we need to replace all four tires and weâre just passed 24,000 miles on our odometer. We are two seniors living in New York State and we only drive on paved roads. We believe this is a failure both of Toyota and Toyota for choosing these tires on this particular model. We were told we need to spend over $1000 to replace tires on a car we do not own. Toyota pointed us in the direction of Toyo, and Toyo pointed us in the direction of Toyota. This is unacceptable and we do not want to pay $1000 for tires that should not be worn out after 24,000 miles with…

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-spe
Source: NHTSA Office of Defects Investigation public data (complaints, manufacturer communications, investigations), model years 2010 and later. Had a safety problem? Report it to NHTSA.
In complaints filed with NHTSA for model years 2017–2025, owners most often name: electrical system, visibility/wiper, unknown or other. 118 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
