On September 20, 2026 at approximately 10:29 a.m. CDT, while traveling on Interstate 30 between Greenville and Royse City, Texas, my 2025 Rivian R1S Gen 2 experienced sustained unintended acceleration in an active construction zone with concrete barriers and no immediate safe exit. I applied very heavy, sustained braking to counter the acceleration until we reached a break in the barriers and could leave the highway. Substantial smoke was then coming from the wheel/brake areas. Backup camera failed to display while backing up. Proceed to drive after leaving home and the display went blank. The failure of the backup camera to display at all was very alarming and unsafe, fortunately no one was around and I was able to safely backup. No warning signs that the display screen was going to go blank. This appears to be related to the most recent software update on my vehicle this month. I have been noticing the screen has been sluggish when attempting to adjust controls.
Safety defect involving exterior rearview mirrors on Rivian R1S I am reporting a recurring and worsening safety issue involving the exterior rearview mirrors on my Rivian R1S Gen 2. Both side mirrors repeatedly lose their saved positions without driver input. More concerningly, on multiple occasions the mirrors have changed position while the vehicle was being driven, including at highway speeds. The specific failed component is UNKNOWN. My 2023 Rivian R1S has experienced recurring intermittent Limited Performance/loss-of-propulsion-type events since within months of delivery. The events involve unexpected reduction of available propulsion, reduced maximum speed, warnings, restarts, system/display restarts, and at times inability to move or change gears. Rivian documented Limited Performance while driving in January 2024. In March 2024, Rivian documented loss of propulsion and an approximately 10 mph maximum speed; a reset and sleep cycle did not correct the condition. OnXXX, at approximately 5:00 PM CT, my 2025 Rivian R1S was involved in a low-speed rear-end collision at the intersection of [XXX] and the [XXX]. I was merging onto the service road (posted speed approx. 55 mph) behind another vehicle that began its merge through a stop sign, then stopped suddenly. I was moving forward at approximately 5-10 mph while checking oncoming traffic to my left. The vehicle's Automatic Emergency Braking system did not engage, and no forward collision warning was displayed, before I struck the vehicle ahead. I applied the brakes manually only after impact. At 56,083 miles, my 2024 Rivian R1S experienced a suspension warning while driving on the US-101 Freeway and entered reduced-performance/turtle mode, limiting the vehicle to approximately 50 MPH while surrounding traffic was traveling substantially faster. The sudden speed differential created a serious highway collision risk. Rivian inspected the vehicle and diagnosed fault P12DF7A, âRoll Control Cross Circuit Static Leakage Check Failure.â The technician repeated the hydraulic leakage test to rule out a false positive. While I was traveling, I suddenly got a brake light fault, turn signal fault, and front camera fault warning messages that appeared on the computer. Rear driver's side tail light/turn signal light stopped functioning and the rest of the brake lights are stuck on, including the rear third tail light while driving and when vehicle is locked/idle. Rivian had me perform soft and hard resets, but that did not solve these problems. Rivian states they are unable to assist me further until I drive it to a service center located near my home.
The contact owns a 2023 Rivian R1S. The contact stated that the vehicle was taken for service, and the service tech stated that all three rear seat belts had a wire that had been severed from the buckle. Additionally, the vehicle failed to detect an occupant seated in the rear seats. The dealer stated that the seat belt buckles needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure, and a complaint was filed. The failure mileage was 29,000.

Rivian is initiating a customer satisfaction campaign for some model year 2025-2026 R1T and R1S vehicles that may be susceptible to windshield washer fluid leaking from the front fascia camera washer nozzle. The customer may notice washer fluid leaking and/or repeated low washer fluid warnings. Rivian will install a check valve in the hose that supplies the front fascia camera washer nozzle. Rivian is initiating a customer satisfaction campaign for some model year 2025 vehicles that may have a loose or cross-threaded ground nut inside the instrument panel. Affected vehicles may not be able to turn on, may not be able to shift into drive or reverse, and may display multiple fault indicator lights on the driver display. Rivian will replace the nut, repair or replace the ground stud, or replace the instrument panel beam. On affected doors, the exterior door handle might not sit flush with the outer door panel or might move slightly when the handle is operated. Upon customer complaint, Rivian will apply new adhesive between the reinforcement and the outer panel.

After detachment, the ultrasonic sensor may become dislodged and hang inside the front fascia, causing customer notifications for sensor issues and potential sensor damage.
