Cracked head cyclinder leaking. Unsafe to drive, coolant can leak into engine if not already. They are aware of it and extended a warranty for the issue in 2024. However people like me, my coolant didn't leak until this year 2026. There have been lawsuits on this issue, but they are still failing to address this issue. They created another part to replace the defected part which costs around $6000-$7000. Yes, took it to a Mazda dealer and confirmed the issue and they can fix but cost would be out of pocket. No warning at all. The only way this was discovered was the smell of the coolant leaking.
Structural bolt (p#: 9-YA01-1401-A) connecting the lower control arm assembly to subframe has rusted apart. The head of the bolt was found in my driveway, otherwise there would be no indication of this potentially catastrophic failure. Vehicle is only 3 years old, was a cpo purchase, and has just 55k miles. Underbody is clean of rust. This is 100% a defect. This is a safety issue. Old bolt will need to be extracted and replaced. Not confident in other bolts so will need to at least inspect those. Hopefully, I can update this will the repair estimate from manufacturer.
My car have a manufacturer engine defect that the company or corporation is not willing to fix. Only because I exceeded the warranty I call them (MAZDA) an told them that it wasn't under my control the these issues happened after the warranty expire. But they still refuse to help me.
While driving at highway speed in heavy rain the oil light suddenly came on followed within 30 seconds by an urgent need to immediately stop the car due to overheating. There was no radiator fluid left due to the sudden and catastrophic structural failure caused by a known manufacturing defect. Mazda calls it csp-11. They did NOT do a recall even though a situation such as this is clearly life-threatening. They offer an extended warranty to fix the problem fully up to 120,000 miles. Our car has 124,000 miles. Two months ago it would have been fully repaired at no cost but now they want in excess of 11 THOUSAND DOLLARS. I have spoken to more than 6 people in customer service, 2 supervisors, …
My 2018 Mazda Cx-9 equipped with the skyactiv-g 2.5T developed a serious cylinder-head/cooling-system failure. The vehicle began to lose coolant with no apparent external leak and subsequently displayed and engine -overheating warning. It continued to lose coolant after a flush. The Mazda dealership inspected the vehicle and diagnosed a warped cylinder head; I was also advised that the engine oil was milky, indicating coolant contamination. The affected component is the engine cylinder head/cooling system. This condition created a safety concern because loss of coolant and overheating can lead to severe engine damage or engine failure and potentially leave the vehicle disabled while driven.…
I entered the interstate normally, with no issues accelerating or merging. While driving at approximately 70 mph, in traffic, all three warning messages appeared on the instrument cluster at the same time: âSmart Brake System Malfunction,â âDriver Attention Alert System Malfunction,â and âForward Smart City Brake Support Malfunction.â At the same moment, the vehicle began to lose speed. I pressed the accelerator pedal firmly. The speed continued to decrease instead of increasing. I could not accelerate at all. An exit was thankfully immediately available. I took the exit, then continued on a side road, then into a parking lot before I could find a safe place to stop. At no poin…
The issue is the steering colomn. There is a bolt that comes loose when driving on "irregular roads". my safety was at risk because if this bolt decided to come out i would not have control of the vehicle. I was told that the steering wheel would disconnect from the steering colomn or the steering wheel would completly detatch. The problem was confirmed and inspected by the Mazda dealership. No warning messages appeared on the dash, only notable symptom was an intense clunking sound coming from behind the steering wheel.
On a high-speed highway (Route 14), the vehicle suffered a sudden flat tire/air loss event, forcing an emergency pull-off onto a narrow shoulder with 65 mph traffic. Subsequent inspection revealed the failed tire and the rest of the set were manufactured in late 2007 (DOT code 4907). These 19-year-old tires were installed by a professional tire shop despite being far beyond their safe service life. Component/System Failed: Tires (Set of 4). Tire Size: 255/50 R20 (Vredestein Wintrac 4 Xtreme, DOT 4907). Available for inspection upon request. Safety Risk: Sudden tire failure at highway speeds created immediate loss-of-control risks and exposed occupants to high-speed semi-truck traffic on a…
Repair instructions and other communications the manufacturer sent dealers about known issues. A TSB is not a recall, and the repair is usually free only while the car is under warranty; ask your dealer for the bulletin number.

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE19-006) on May 2, 2019 to investigate two Vehicle Owner Questionnaires (VOQs) and Early Warning Reporting data related to allegations of inadvertent deployment (IAD) of side curtain airbags in Model Year (MY) 2010 through 2013 Mazda CX-9 vehicles. ODI sent an Information Request (IR) to Mazda North America Operations, LLC. to gather information about the alleged defect in the subject vehicles. In the 20 months since
In an July 7,2015 letter to NHTSA, Mazda identified a safety defect that could result in lower ball joint separation in approximately 193,484 model year (MY) 2007 through 2014 Mazda CX-9 vehicles (NHTSA Recall No. 15V-451,Mazda 8515G).Mazda indicated that damage to the ball joint boot may allow water to enter the front suspension ball joint fitting during use which may result in premature ball joint wear and a progressively looser joint. In January 2014, Mazda implemented redesigned lower control
In March 2014, Mazda Motor Corporation (Mazda) initiated Special Service Program SSP93 to extend the warranty coverage for a specific brake booster fault condition on model year (MY) 2007-2013 Mazda CX-9 vehicles.According to Mazda's dealer communication for SSP93, the subject vehicles may exhibit a condition were the brake pedal is harder than usual to depress. Complainants report hearing air leakage (hissing) from the driver-side foot area during braking. The Mazda program extends the warranty
Source: NHTSA Office of Defects Investigation public data (complaints, manufacturer communications, investigations), model years 2010 and later. Had a safety problem? Report it to NHTSA.
In complaints filed with NHTSA for model years 2010–2023, owners most often name: service brakes, engine, power train. 1,139 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
