The contact owns a 2018 Lexus ES350. The contact stated that while driving approximately 5 MPH and making a left turn into a parking lot, the vehicle suddenly experienced unintended acceleration driving to the opposite side of the parking lot before the vehicle shut off. The contact stated that moments later the vehicle was restarted and operated normally. The vehicle was taken to the local dealer who was unable to duplicate the failure. The mechanic was unable to determine the cause of the failure. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 66,150.

While driving my 2021 Lexus ES 350 on a Interstate Highway 495 known as the Long Island Expressway my vehicle went over a small pot hole in the road way and the tires side wall was punctured resulting in a flat tire. I replaced the tire having only 4,555 miles on this new vehicle. Following this incident I once again drove over obstruction more than 4 inches and had both the drivers side tire and the drivers side rear tire side walls punctured with the additional damage of the drivers side front rim so badly damaged it had to be replaced. The left headlight projects three dark shadows on the road surface along the left edge and middle of the road in front of the vehicle Someone walking or standing on the side might not be visible and when the vehicle is turning to the right the shadows flow along the very edge of the roadway obscuring anything in its path. The car has been to the dealership twice and they verified that the shadows due exist and are operating as intended.

In addition, some models may also utilize retaining hooks (clips) for passenger side and rear seat floor mats. During Pre-Delivery Service (PDS), follow the Installation Procedure in this bulletin to attach the driver side, and if applicable, the passenger side and rear seat floor mats to the vehicle carpet using the floor mat retaining hooks (clips).

DL: On December 20, 2022, a settlement of claims for certain low pressure fuel pumps was approved by the federal court. The settlement includes Lexus?s agreement to provide a Customer Support Program for certain 2013-2020 model year (MY) vehicles originally equipped with low pressure fuel pumps repaired under Safety Recall 20LA01 or Special Service Campaign 21LC01. The purpose of this Customer Support Program letter is to help clarify how to administer this coverage in accordance with the settlement.

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 20
