Vehicle stalled and had a power system failure requiring driver to pull off to the side of a road that blocked traffic. The acceleration/drive function no longer worked. The vehicle displayed a POWER SYSTEM PROBLEM message on the dashboard and showed "DO NOT DRIVE". Vehicle was towed to HONDA dealership and they confirmed the fuel pump needs to be replaced. The vehicle had a replacement of the fuel pump in 2022 as part of a prior recall, but the same component is responsible for this situation four years later. On August 23, 2026, the electronic power steering and braking systems on a 2025 Honda Accord Hybrid completely failed and locked up simultaneously while traveling at highway speeds. This caused an immediate and severe safety risk, resulting in a near-collision on a busy freeway with three occupants inside the vehicle. The vehicle is currently available for inspection upon request. Prior to and during the failure, widespread dashboard warning lights illuminated simultaneously, including the Collision Mitigation Braking System, Lane Keeping Assist System, Road Departure Mitigation, and Low Speed Braking Control. The vehicle has a documented, severe manufacturing defect involving a cylinder head gasket leak under Honda Service Bulletin 26-006. A certified technician at Honda East Cincinnati confirmed the head gasket leak in writing on September 9, 2026. Prior to this, an independent facility pulled code P0302 (Cylinder 2 Misfire) and used a digital borescope to capture video proof of liquid coolant pooling inside Cylinder #2. While driving on a bridge at under 20 mph, the vehicle's head gasket failed, causing the engine to overheat and coolant to leak from within the engine. The vehicle threw diagnostic trouble code P2601 and immediately entered "limp mode," severely cutting engine power and vehicle speed. Because this happened on a bridge with no shoulder to safely pull over, the sudden loss of power created a serious safety hazard â the vehicle could not maintain a safe speed or keep pace with surrounding traffic, increasing the risk of being struck by other vehicles. Most recent incidents on Friday 08/07 car lost power while driving on freeway unable to accelerate, all engine/warning lights came on was thankfully able to pullover and shut off vehicle. This is not first incident this also happened about 5x in 2025 and had car inspected and serviced by Honda dealership each time being told they didnât know what the issue is because the engine light wasnât on at time of appointments. The contact owns a 2024 Honda Accord Hybrid. The contact stated that while driving at an undisclosed speed into a parking space, the brake pedal was depressed but the vehicle failed to stop. The vehicle unintendedly accelerated and drove over a parking space bump. The contact shifted the transmission into reverse(R) and depressed the accelerator pedal, and then the brake pedal, but the vehicle failed to stop. The engine failed catastrophically. White smoke out of the exhaust at 145,000 miles. Dealer checked out the car and they initially though it was a head gasket failure. After taking the engine head off, it turns out it was a 'structural defect' (verbatim words from the mechanic), which was a divot in the engine block. The head gasket was in great shape. Therefore, I needed a 'new' used engine (at $9K) instead of a head gasket replacement (at $4K). A search on the internet shows that forums (driveaccord.net, facebook groups) shows that many Honda Accord owners have similar head gasket failures. I contacted Honda USA and they said there's no recall so I won't get reimbursed.
