On two separate occasions, the vehicle's left rear wheel studs have broken off clean with the hub and both rear dual wheels departed the vehicle while in motion. This issue was reported to the local FORD dealer and they reported that they do not know why this is happening. This has happened to five (5) of our other vehicles as well. I am a fleet manager for an EMS service and we have a total of ten (10) of these Ford chassis. The wheels coming off present a hazard to oncoming traffic not to mention the safety of our team members and patients being transported on our ambulances. The L shaped bracket that supports the RV hydraulic/motor assembly for the auto leveling system has failed. The weld between two pieces of steel that makes up the bracket has failed. When the bracket fails the assembly is supported temporarily by electrical wires and hydraulic hoses. If it fails further and drags the road or separates completely, there is a possibility of fire under the vehicle and/or death or injury to other drivers on the road. There other safety recalls for this exact failure but my unit was not listed.
Description When your vehicle is in reverse gear, the vehicles exterior sounder backup alarm may only produce an intermittent warning sound or may be inoperative and produce no warning sound. Safety Risk A backup alarm that does not emit sound may fail to alert pedestrians or other road users of a reversing vehicle, increasing the risk of a crash or injury. Parking Brake The Sticker on this U-Haul 15 foot Truck says, "Engage the Parking Brake before turning off the Engine, otherwise UNCONTROLLED OPERATION OF VEHICLE may occur.". I pulled on the Parking Brake as the Sticker warned. With my foot off the Gas Pedal, at about 25 - 30 MPH the U-Haul Steered way to the right by itself, and drove onto the Right Curb. The contact rented a 2016 U-Haul E450. The contact stated that while in the parking lot and placing the vehicle in drive, the contact tapped on the accelerator pedal, then when the brakes were applied and the vehicle suddenly accelerated. The contact removed her foot from the brake, and it continued to accelerate and crashed into a parked U-Haul truck. There were no reported injuries, fire, airbag deployment, or police report filed. Lifters in engine is going out. Ford has made a TSBs for repair because lifters and camshaft was made by a third party and they are defect. Here is Technical Service Bulletin (TSB 25-2200). Vehicle is available for inspection. It was put at risk because vehicle could loose power at any time and potential crash into another car. The problem has been reproduced in 4 different Ford vehicles. Has not been inspected by manufacturer or others. No warning lights were present.
Engine camshaft and lifters failed. Truck is at a Ford dealer. Engine could fail in the middle of the road, causing an accident. This is our eighth vehicle with the same issue taken to a ford dealer in the past year. A little knocking noise started that quickly turned into a loud knocking/rattling noise. Ford and Lincoln vehicles equipped with wired keyless entry keypad systems and accessory wireless keyless entry keypad systems may or may not come with a wallet card containing the master code. Unlike the integrated wired keypad, the accessory wireless keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool or from the label printed on the body control module (BCM).
