Component: Driver Monitor Camera (DMC) â driver attention/drowsiness monitoring system. The Driver Monitor Camera repeatedly and incorrectly determines that the driver's eyes are closed when they are fully open and the driver is attentive and watching the road. This occurs during normal, attentive driving. When the system falsely registers "eyes closed," it issues escalating visual and audible alerts that grow progressively louder. The only reliable way to silence the escalating alarm is to briefly close one's eyes â meaning the system induces the exact unsafe behavior (eyes off the road) that a driver-attention system is supposed to prevent. This is a significant distraction and safet…
A vehicle was sold and delivered while under an active stopâsale safety recall (manufacturer recall 26TB03). The recall was not disclosed at the time of purchase. When questions were raised about whether any recalls were present, the issue was dismissed and no recall information was provided. According to the recall documentation, the potential defect involves the rear door system possibly opening while the vehicle is in motion. Because the stopâsale indicates the vehicle should not have been sold, the safety risk is that the vehicle was driven with occupants in the rear seats without anyone being informed of the potential hazard. This created a risk to the people inside the vehicle and…
The contact owns a 2025 Toyota Prius PLUG-IN Hybrid. The contact stated that while driving at various speeds, the rear passenger door would open without warning. The contact then received notification of NHTSA Campaign Number: 26V049000 (ELECTRICAL SYSTEM, LATCHES/LOCKS/LINKAGES) however, the part to do the recall repair was not yet available. The local dealer was made aware of the issue and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The failure mileage was approximately 3,000.
Infotainment screen failed to boot when the vehicle was turned on. Due to this failure, the rear view camera was not available when the vehicle was shifted to reverse. After backing out of the parking spot, I chose to park and restart the vehicle. The infotainment screen then proceeded to boot up and behave normally. There were no warnings or any other alerts or messages. A disabled rear view camera can impact pedestrian and vehicle safety by limiting rear vision while operating the vehicle in reverse. During the drive home, approximately 15 minutes later, the infotainment system went to a black screen and rebooted. After 10-15 seconds, the system had restarted and was available a…
The recall (Number 26V049) is a serious safety concern, however the dealer states there is no remedy available despite a clearly described available remedy ("Toyota dealers will modify the left and right rear door switch circuits FREE OF CHARGE, to prevent the switch from activating even if it is shorted.")
I was driving at a slow speed in a parking lot on October 10, 2025 at 4:20 p.m. when the car suddenly stopped. When I looked down I saw that it was in neutral. I had not done anything to the gear shift and there was nothing in the area that could have shifted the car from drive to neutral. The car is brand new - this happened at approximately 400 miles odometer.
The pearl white paint on my 2015 Toyota Prius plug in vehicle is starting to chip and delaminate. This poses a safetly hazard to the structural rigidity of the body which could rust, break down, and fail unexpectedly. I am requesting to hold Toyota, who has had paint issues with their vehicles in the past, liable and to pay for the cost of a new paint job. Please help!
The contact owns a 2014 Toyota Prius Plug-In-Hybrid. The contact stated while driving 65 MPH, the brake pedal was depressed; however, the vehicle failed to stop as intended. The contact stated that the brake pedal had to be depressed abnormally hard to stop the vehicle. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure. The contact was informed that the VIN was not under recall. The failure mileage was unknown.
Repair instructions and other communications the manufacturer sent dealers about known issues. A TSB is not a recall, and the repair is usually free only while the car is under warranty; ask your dealer for the bulletin number.
This bulletin contains the PDS procedures that apply specifically to the 2027 model year Prius and Prius PHV vehicles
2026 model year Toyota vehicles are equipped with driver side floor mat retainers to secure the floor mat in the correct position. In addition, some models may also utilize retainers for passenger side and rear seat floor mats. During Pre-Delivery Service (PDS), follow the appropriate Installation Procedure to attach the driver side, and if applicable, the passenger side and rear seatfloor mats to the vehicle carpet using the floor mat retainers.
During the manufacturing process, tires are mounted on the vehicle as matched sets to help reduce conditions such as pulling or drifting. If the wheels are replaced or refinished before vehicle delivery, it is important that the tires remain on the vehicle in the same location and in the same direction of rotation.
A battery in a stored vehicle is subject to conditions that can reduce its performance and service life. These conditions include storage period, temperature, parasitic drain, and battery load. Because of these factors, battery inspection and maintenance are required in order to ensure proper operation and optimal battery life.
Long-term or off-site storage requires special care to maintain vehicle condition. The following guidelines should be performed to minimize vehicle component/part degradation due to extendedvehicle storage conditions. Long-term storage can affect a vehicleâs systems and components. Any problems that are found should be corrected immediately.
Pre-Delivery Service (PDS) is a critical step in satisfying our new car customers. Customer feedback indicates the following areas deserve special attention when performing PDS: ⢠Careful inspection for paint chips/scratches and body dents/dings. ⢠Proper operation of electrical accessories (including interior light, clock, and radio reset). ⢠Interior cleanliness. ⢠Proper function of mechanical systems. The PDS process helps identify potential vehicle concerns before customer delivery. After completing the PDS process, maintain accurate PDS records by ensuring the required docume
Some Customers for the above vehicles may have complaint of the following: unable to charge the vehicle using the supplied level 1 or 2 charger. Additionally, most will indicate one or more of the LEDs on the charger will illuminate at the time of no charge. Charging at other stations is still possible.

NHTSA’s Office of Defects Investigation opens these when complaints point to a possible safety defect. Many end in a recall.
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-spe
On September 19, 2019, the Office of Defects Investigation (ODI) received a defect petition from Mr. Roger Hogan requesting that the Agency investigate and recall certain Toyota Prius, Prius PHV, Camry Hybrid and Avalon Hybrid vehicles for a brake actuator valve wear condition covered under Toyota Customer Support Programs ZJB and ZKK (subject CSP?s). In support of his request, the petitioner identified 117 NHTSA complaints allegedly related to the subject CSP condition, including 60 reporting c

In complaints filed with NHTSA for model years 2010–2027, owners most often name: service brakes, electrical system, unknown or other. 68 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
