What MAZDA MAZDA5 owners have reported to NHTSA, the service bulletins the manufacturer sent dealers, and any federal defect investigations, by model year. See MAZDA MAZDA5 safety recalls or check your VIN.
Complaints owners filed with NHTSA, by the model year of the vehicle. A high count can also mean a popular year, so compare it with how many were sold.

The contact owns a 2015 Mazda Mazda5. The contact stated that while parked with the vehicle occupied, the contact became aware that the rear driverâs and passengerâs side seats were melting. A dealer was contacted. The vehicle was not taken to the dealer or an independent mechanic to be diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 130,000.
While accelerating from a complete stop at a traffic light on a multi-lane roadway, the vehicle suddenly lost all engine power without prior warning. The engine RPM dropped immediately to 0, causing the vehicle to stall in the middle of an active intersection and creating an immediate risk of a side-impact collision from oncoming traffic. All warning lights on the instrument cluster illuminated upon stalling. To clear the intersection safely, the vehicle was placed into Neutral and restarted. After shifting back into Drive and proceeding out of traffic, the vehicle repeatedly entered a fail-safe/limp mode every 30 to 120 seconds. During these episodes, the Automatic Transaxle (AT) light fla…
The handle on the sliding door and driver's side broke off. We have replaced it twice already and it keeps breaking. This has not happened to me with any of my other cars in the past or present until now. It's unkown to me and it shows the part was not done well. What caught my attention is that I saw another mazda5 minivan without a handle which made me think that this is a problem with this type of car. I'm surprised it doesn't have a recall. Please help us with this. Thank you.
The contact owned a 2012 Mazda Mazda5. The contact stated that while driving approximately 45 MPH, a deer wandered into the middle of the road, and the contact depressed the brake pedal and the wheels locked up, causing the tires to slide. The contact lost control of the vehicle and crashed into a ditch, spun around, and struck another object, causing the vehicle to land back into the ditch where it came to a stop. The air bags did not deploy upon impact. A bystander called 911, and the contact was taken to the hospital by ambulance. The contact bit his lips hard during the crash, which caused the bottom two front teeth to pierce through the bottom lip. The contact sustained a severe cut to …
All of the door handles and locks have loosened and fallen off. Sliding door will not latch now. Hatch on rear has lost suspension.
I bought this vehicle as is, about 2 months past, after driving it home I felt after pressing the Brakes pedal that it is not responding until I press 2 or 3 times. I went bnack to the dealer the next day and made a complain. They sent me to their Mechanic who diagnosed the problem to be the ABS Hydraulic Control Unit Assembly. It is available for inspection upon request. My safety and the safety of my family was put at risk because of the fact that I have to pump the brakes to slow the vehicle. Yes the problem was confirmed by the independent Mechanic recomended by the dealer, with a detailed estimate. No inspection was performed by the manufacturer, police or insurance representatives. Sentury Touring (Discount Tire) 205/55/R16. Both front tires have inner wear to the point of shredding in the same location on *both* front tires. Both tires went out at the same time and could have caused a serious accident. Discount tire stated it was an alignment issue. The alignment shop (30 minutes after new tires were installed after dual blowout) stated that my alignment was perfectly within spec and also stated there was no indication of tire rub on any portion of the vehicle.
On several occasions, most of which happened while in motion, my vehicle has stalled out of the blue with no warning. It seems to mostly happen when Iâm slowing down or when Iâm making a right hand turn and try to accelerate, suddenly thereâs no power to the engine. I have to put the vehicle into park and restart it. Twice while slowing down for a red light. Twice immediately after making a right handed turn, and once, when entering roundabout, after yielding. This is happened at least five times. The most recent incident occurring in August 2023.
Repair instructions and other communications the manufacturer sent dealers about known issues. A TSB is not a recall, and the repair is usually free only while the car is under warranty; ask your dealer for the bulletin number.

A musty/mildew type odor may come from the vents when the air conditioning (A/C) system is operating. It is most noticeable when the A/C is first turned ON. This odor is the result of mold growth in the A/C evaporator/cooling unit which is caused by condensation, dust, and pollen within the cooling unit. This condition is usually worse during high humidity conditions. âMazda Air Cooling Coil Coatingâ is available to encapsulate the mold to reduce odors. If the product is properly applied, it can effectively reduce the musty/mildew odor for up to three years. NOTE: See parts information for
Some vehicles may experience exterior trim heat damage such (melted, separating, etc.) when parked near a glass building or Low-E windows. This may be caused by the sun's reflection. If a structure's windows create enough of a curve, the concave glass can focus the sun's rays onto a small area and acting like magnifiying glass direct the sunbeams into a hot pinpoint of light. This magnifcation of the sun's rays can cause damage to vehicle trim (see examples). Additional information and exemplar images regarding this phenomenon can be found by performing a google search.
When replacing the SiriusXM satellite radio unit, a call must be made to inform SiriusXM of the existing Radio ID and the new/replacement SAT Radio ID as soon as possible to ensure the customer leaves the dealership with an active SAT radio.
Some customers may complain about a noisy HVAC blower motor or odor after a cabin air filter replacement. Foreign debris on the blower motor after servicing the cabin air filter may be the cause. During the removal of the cabin air filter, foreign debris (leaves, dust, dirt....ect.) may fall on the blower motor causing a fluttering type noise or odor.

The SKYACTIV-DRIVE FW6A-EL AT uses âAutomatic Transmission Fluid (ATF) Type: FZâ. Only use âATF-FZâ to fill or top off the fluid level. ATF-FZ is specially designed for use in the SKYACTIV-DRIVE series AT. It is a low viscosity formula with less friction for better fuel economy. NOTE: ATF-FZ is colored blue for easy identification.
Some customers may complain about scratches on the instrument cluster lens.
Source: NHTSA Office of Defects Investigation public data (complaints, manufacturer communications, investigations), model years 2010 and later. Had a safety problem? Report it to NHTSA.
In complaints filed with NHTSA for model years 2010–2018, owners most often name: air bags, structure, engine. 142 complaints are on record.
No. A complaint is an unverified report from an owner. A technical service bulletin (TSB) is the manufacturer telling dealers how to fix a known issue; it is not a safety recall and is usually covered only under warranty. A recall is a formal safety action, and its repair is free.
File a vehicle safety complaint at NHTSA.gov. NHTSA reads every report, and patterns of complaints are what open defect investigations and, eventually, recalls.
